Eastern NC Health Stewards are a group of stakeholders looking for ways to collaborate and share resources to improve health of the population in eastern NC. These efforts are supported by a Steering Committee, with Dr. Ron Gaskins currently serving as Chair. The Steering Committee is tasked with exploring and pursue grant opportunities to impact/improve health in eastern North Carolina. Member organizations of the Eastern NC Health Stewards support the Steering Committee in identifying and pursuing opportunities for eastern North Carolina.
One of the first projects the Easter NC Health Stewards are pursing is the Healthy Opportunities Pilots. North Carolina Department of Health and Human Services is launching the Healthy Opportunities Pilots as part of its Medicaid Transformation efforts. The pilots will be an innovative opportunity, through the State’s Medicaid 1115 waiver, that will allow the state to test and evaluate the impact of providing select evidence-based, non-medical interventions related to housing, food, transportation and interpersonal safety to high-needs Medicaid enrollees. The Steering Committee is currently preparing the Stewards to respond to the Healthy Opportunities Pilots request for proposals (RFP) when it is released in fall 2019.
Access East has been selected by the Eastern NC Health Stewards to serve the lead pilot entity (LPE) for their application. It is expected that DHHS will select two to four LPEs in early 2020 from the RFP process. A key LPE responsibility will be to develop a network of HSOs in its region that will deliver Pilot services to Pilot enrollees.
In addition, the Stewards are working with NCCARE360 to assist with the onboarding of eastern NC counties to the resource platform. NCCARE360 is a statewide coordinated care network of health care and human services organizations through a shared statewide technology platform that allows for a community-oriented, person-centered approach to delivering care in North Carolina.
